Frequently Asked Questions
Your first visit is a focused consultation — typically 45–60 minutes. Dr. York will review your imaging, perform a thorough physical examination, and discuss your history in detail. He will explain his findings clearly, present all treatment options (non-operative and operative), and answer every question you bring. There is no rushed assembly-line experience. Bring your imaging films or DICOM files, a list of current medications, and your completed New Patient Intake Form if possible.
New patient consultations are typically available within 1–2 weeks. For urgent or time-sensitive cases — including workers' compensation, pre-operative second opinions, or post-injury evaluations — Dr. York's team will make every effort to accommodate you sooner. AXIS is a direct-pay, concierge orthopedic practice. You pay Dr. York directly — no insurance middlemen, no surprise bills. Fees are transparent and disclosed upfront. The Patient Payment Guide (available in the downloads above) details consultation, procedure, and surgical fees. Most patients find that cash-pay pricing at AXIS is competitive with or lower than their out-of-pocket costs under insurance, especially for high-deductible plans.
Yes — HSA and FSA cards are accepted for all services. AXIS does not bill insurance directly, but we will provide you with an itemized superbill (receipt with procedure codes) that you can submit to your insurance company for potential out-of-network reimbursement. Many PPO plans reimburse a portion of out-of-network surgical and consultation fees. We recommend calling your insurer in advance to understand your out-of-network benefits.
Yes. Dr. York has opted out of Medicare pursuant to 42 CFR § 405.415. This means Medicare will not pay for his services, but you can still receive care from him by signing a Medicare Opt-Out Private Contract (available in the Legal & Administrative tab). You pay Dr. York directly. Medicare Advantage and Medigap plans are also prohibited from paying for services under a private contract. If you prefer to stay within the Medicare system, Dr. York can refer you to a participating provider.
Federal law (42 U.S.C. § 1395a) requires that any Medicare beneficiary who receives services from an opted-out physician must sign a written private contract acknowledging that Medicare will not pay for those services and that they are responsible for the full fee. This is a legal requirement — not a waiver of your rights. You retain the right to seek care from any Medicare-participating provider at any time. For elective joint replacement, the typical timeline is 2–6 weeks from consultation to surgery, depending on pre-operative clearance, anesthesia scheduling, and implant availability. Dr. York performs surgery at HonorHealth Greenbaum Surgical Hospital in Scottsdale. Pre-operative appointments, lab work, and anesthesia consultation are coordinated by his team. Urgent or time-sensitive cases can often be expedited.

Most joint replacement procedures are performed as same-day or 23-hour stays. The SuperPATH hip approach and kinematic alignment knee technique are specifically designed for faster recovery, reduced pain, and earlier mobilization compared to traditional approaches.
Kinematic alignment (KA) is a patient-specific approach to total knee replacement that restores the natural alignment of your individual knee — preserving the original ligament tension and joint line geometry. Traditional mechanical alignment (MA) uses a standardized, population-average alignment that may not match your anatomy. An IME is an objective medical evaluation performed by a physician who has no prior treating relationship with the examinee. IMEs are used in workers' compensation claims, personal injury litigation, disability determinations, and insurance disputes to provide an independent opinion on diagnosis, causation, treatment necessity, and impairment rating. Dr. York conducts IMEs for attorneys, insurance carriers, and employers throughout Arizona. See the IME section of this site for referral information and fee schedules.
Download the IME Referral Checklist from the Legal & Administrative tab and submit it along with all available medical records, imaging, and claim documentation to [email protected]. A signed IME Retainer Agreement and the $1,750 non-refundable retainer are required to confirm the appointment. Reports are typically delivered within 10–15 business days of the examination. Rush turnaround is available for an additional fee.

Medicare Status
Dr. York has formally opted out of the Medicare program. This means AXIS does not bill Medicare for any services, and Medicare will not reimburse patients for care received here.
If you are a Medicare beneficiary and choose to receive care at AXIS, you will be asked to sign a Medicare Private Contract — a federally required document that acknowledges you understand no Medicare reimbursement will occur and that you accept full financial responsibility.
This is a standard requirement for any opted-out physician in the United States. It does not affect your Medicare eligibility for care received elsewhere.
Opting out of Medicare does not affect Dr. York's ability to see patients with private insurance or personal injury cases. AXIS is a cash-pay practice — all patients pay directly regardless of insurance status.
Medical-Legal Services
Dr. York provides orthopedic medical-legal opinions for attorneys, insurers, and employers — serving both plaintiff and defense equally. With 25+ years of clinical experience across the full spectrum of orthopedic conditions, he is qualified to opine on most musculoskeletal body parts.
Thorough history, physical examination, and review of all records and imaging. Opinions on causation, MMI, permanent impairment, and estimated future orthopedic care costs.
AMA Guides 5th and 6th Edition disability ratings. Required forms completed as part of the engagement. CIME/CICE certified (August 2024).
